– do wykonania przez spółkę Leo Apartments Sp. z o.o. usługi hotelowej niezbędne jest przetwarzanie przez nią danych osobowych osób korzystających z miejsc noclegowych, a ponadto osób zawierających umowę o świadczenie usługi noclegowej (jeżeli to inne osoby), a obejmujących ich imię, nazwisko, PESEL lub inny numer identyfikacyjny osoby, adres zamieszkania, inne dane kontaktowe (telefon, e-mail) i decydując się na dalsze zawieranie i/lub wykonywanie umowy, nie sprzeciwiam się temu;
– in the provision, by limited liability company Leo Apartments Sp. z o.o. , of a hotel service, it is necessary for Leo Apartments Sp. z o.o. to process the personal data of the individuals using the hotel accommodation service provided by him, as well as the personal data of any individual bound by a contract for the provision of the hotel service (if these are not the same individuals), including in each case, their first names, surnames, PESEL [national ID numbers] or other identification numbers, addresses of residence, other contract details (phone numbers, e-mail addresses), and I do not object to such processing if I continue to enter into and/or use such services under such a contract;
– przetwarzanie to ma miejsce w celach związanych z zawieraniem, wykonywaniem i udokumentowaniem wykonywania umowy, przez czas jej wykonania, rozliczenia, a następnie do czasu ustania obowiązków związanych z taką umową oraz wygaśnięcia roszczeń z jej tytułu, przy czym okres przetwarzania danych znajduje swoją podstawę prawną zwłaszcza w przepisach o przedawnieniu roszczeń (art. 118 i 442 ustawy Kodeks cywilny), jak również nakazujących przechowywanie dowodów księgowych (art. 74 ust. 2 ustawy o rachunkowości);

– W związku z tym, że przetwarzanie danych osobowych opisanych bezpośrednio wyżej ma miejsce wyłącznie na podstawie mojej zgody, oświadczam, że przyjąłem do wiadomości ponadto, iż mogę taką zgodę cofnąć w dowolnym momencie, bez wpływu na zgodność z prawem przetwarzania danych sprzed cofnięcia.
